Medical forms are printed in early stage of the process (well before assessment), so it is quite customary that they show your old address as you changed it quite a short time ago. Medical forms are only valid for 1 year from the date it was signed by the physician. Therefore, a new form is required each year. Free medical consent forms are be completed by parents before children can be included in an excursion and must accompany the teacher on the excursion. If an excursion involves water activities parents must indicate the swimming ability of their child when giving consent for their child to participate in the excursion. Medical forms are then assesed by the Council’s medical advisor. Medical Forms are imperative 30 days prior to your session date.
